Interest Participation Form

Iowa e-Health is seeking information from provider organizations that may be interested in participating in the IHIN. The estimated time frame for the Direct Secure Messaging phase of roll-out is August - September 2012.  Completion of this form will help Iowa e-Health plan an appropriate implementation schedule and communicate with all interested organizations.  Contact us with questions related to the Participation Interest Form.
